Your First Visit
Save time when you come to see us by downloading and filling out the forms before your first visit.
A careful and comprehensive retinal exam can be time consuming. Please prepare to be in our office for up to 2 hours. Additional testing such as fluorescein angiography may extend the visit even longer. During the initial visit, we will ask you questions about your eyes, your general health, and any medications you take. Both of your eyes may be dilated for the exam. Please bring a companion to drive you home after the exam.
Once you arrive at the offices of Vitreoretinal Consultants of NY, you will be greeted by a member of our Check-In staff who will ask you to fill out a patient questionnaire. This is important as we strive to keep our files as current as possible. You will also be asked to provide us with your insurance ID cards, as we keep copies of these as part of your chart.
Once you are checked in, one of our ophthalmic technicians will do a preliminary eye exam and measure your vision. Your pressure will be checked and your eyes will be dilated.
After your eyes dilate you will be examined by one of our physicians. We will use several techniques to fully evaluate your macula, vitreous and peripheral retina. Following the exam additional testing or treatments may be recommended. Follow up exams or treatments are often necessary to evaluate treatment or changes in your condition. We look forward to working with you to ensure your experience with us is productive and helpful.
What you should bring to your first visit:
- Photo ID
- Insurance Card
- Referral or authorization from your insurance, if needed
- First-time visitors' forms (see above)
